SeniorCare’s Elder Care Advisor can help with:

• In home assessment
• Short term care advice
• Education on every aspect of elder care
• Planning for elder care needs
• Assistance navigating the elder care system
• Referrals to elder services and programs

SeniorCare’s Elder Care Advisor can visit your home to consult with you to plan for elder care for you or a loved one.

The Advisor will provide you with assessment and advice, refer you to resources, and be available for short-term follow up.  Assessment and referral services are free to elders and their family members within the SeniorCare service area.
